SMP I & II Burn Cancellation Form Instructions

If for any reason a burn project is cancelled, the Air Quality Bureau (AQB) needs to be notified of this cancellation as soon as possible. This form may be submitted on-line (http://smoke.state.nm.us) or to the AQB by fax at (505) 476-4375, phone (1-800-224-7009) or e-mail (NMENV.smoke@state.nm.us).

Explanation: This information is needed for daily airshed assessments. 

Burn Project Name.  Enter the name of the Project.

Burn Project ID.  Enter the ID number assigned by the AQB from the initial registration.

Date Canceled.  Enter the date the burn project has been cancelled. Only one date per form.

Contact Name. Enter the name of the person responsible for the burn project.

Phone Number. Enter the phone number for the contact person.

Comments. Enter any other comments that might help clarify or explain other entries.
